It was recently shown that serum concentrations of 25-hydroxycholecalciferol (25-hydroxyvitamin D; 25(OH)D) fell by more than 40% within 24 hours of elective joint replacement surgery and were still 20% lower than pre-operative values three months after the operation.1 Quantifi cation of serum 25(OH)D concentrations has provided us with a measure of vitamin D status for more than 40 years. The pioneering work of David Frazer, Eric Lawson, Egan Kodicek, Michael Holick and Hector DeLuca in the 1970s identified 25-hydroxyvitamin D and 1,25-dihydroxyvitamin D; they showed us that 25(OH)D and 1,25 dihydroxy-cholecalciferol (1,25(OH)₂D) were the two major metabolites of vitamin D in our blood controlling calcium and phosphate metabolism. Subsequent work
showed that the plasma concentration of 25(OH)D was an accurate indication of vitamin D stores, whether obtained fr om ultraviolet irradiation or dietary intake over long periods.
